Referees should report their game results using the following website:
The following information should be included in your Game Reports:
Date, time and site of game.
Home and visiting team names.
Final score.
Select if the center referee and assistant referees arrived on time, late or were a no show.
Note the names of any players that did not play due to suspension.
Goal scorers - include the team name, full name as it appears on the roster, jersey number and time.
Carded players (Yellow or Red) - include the team name, full name as it appears on the roster, jersey number, time and reason.
If you have a send off, indicate briefly in the report comments section, as well as, sending the Referee Assignor a completed USSF Supplemental Game Report withing 48 hours of the issuance of any sendoff.
Please keep all paperwork (rosters, reports, etc.) from games for future reference in case of any appeals or other circumstances that may come up.
Please include any notes about violation of team requirements, such as home team not having corner flags or a game ball, teams not having rosters or player passes, etc. in the comments section of the Game Report. This is imperative to allow CSL to have documentation in order to enforce sanctions on teams who are habitual offenders.

CSL will keep a list of all players suspended for the coming weekend's games. Only players with the date of the game you will be refereeing listed in the column labeled "Date(s) Susp." should not be allowed to play.
